VETERINARIAN SERVICE PROVIDER AGREEMENT





As a condition of membership, all our providers sign an agreement of quality care and standards with Affordable Vet Referral Service. This is part of our provider agreement.

◊Provider shall provide high quality service to all clients referred by Affordable Vet Referral Service as it would any other client.

◊Provider shall treat all animals referred by Affordable Veterinary Referral Service humanely.

◊Provider shall honor estimate provided during the bidding process, as is possible.

◊Provider shall notify client of any anticipated issue that may arise before, during or after the treatment, prior to treating the patient.

◊Provider shall notify client of any unanticipated costs prior to treating the patient.

◊Provider shall practice good ethics when speaking of other veterinarians, including client's current regular veterinary.

◊Provider shall work closely with the client and patient, maintaining client notified of patient's progress.

◊Provider shall provide care for the patient until the treatment is fully completed (i.e. rechecks, suture removals, etc.).

◊Provider shall notify client if patient needs to remain under observation for 24 hours.

◊Provider shall provide options to care for patients needing 24 hours care when overnight care is not available at provider's location.
